Pickleball 10-Visit Pass Enrolment Form

Terms and Conditions:
• Visit passes are valid for 3 years from the date of purchase. No further extensions apply.
• Visit passes are non-refundable.
• Key Tags are issued at the time of purchase and must be scanned to gain entry. One scan per visitor is required at Reception. Lost cards will incur a $6 replacement fee.
• Visit Pass is valid at either The Rise or Morley Sport & Recreation.
• Session Bookings must still be made via Team Reach
• Passes must be used in a like for like capacity (Purchased AM sessions can only be used for AM sessions.
PM sessions are only valid for PM sessions).
