This Induction is to be completed on first visit to each Recreation Facility and repeated every 12 months (once per calendar year) or as information is updated.

On arrival, please see reception to sign in. The Customer Service team will alert the Duty Supervisor, Recreation Attendant or the issuing Officer of your arrival. Please sign out and return any Bayswater equipment on exit.
Is the visitor aware of any potential hazards and risks associated with the visit and/or working on this site?(Required)
Site Specific Hazards Dangerous Goods- Bayswater Waves is a licensed facility storing and utilising dangerous goods for pool water treatment. Please be aware that entry to the Chlorine Gas store is only permitted to qualified operators and emergency services or under the direct supervision of a qualified operator. Chlorine Gas pipework under vacuum exists external to the building and in plant rooms. These lines are marked Yellow with Black writing. Child Minding Services- Bayswater Waves and The RISE operates a creche facility. Works and visitors are not to be completed in this area during. Food Services- Bayswater Waves hosts a Café operated by a third-party company. Approval to enter this area is required by the Café staff. The RISE and Morley Sport facilities include food preparation and serving areas. Please be aware of food preparation areas and report any contamination to Café staff for cleaning.

Emergency Assembly In the event of an emergency requiring an evacuation, a ‘whoop, whoop’ alarm will activate. The Primary Emergency Assembly route via front reception. Alternative Assembly Areas are available depending on the type and location of the emergency. Facility staff will direct you to the required Evacuation Assembly Area.
Has it been explained to the visitor what to do if you become separated and how to obtain treatment or assistance?(Required)
Assistance Required Where assistance is required or you are separated from your host. Please speak to a Lifeguard on Pool Deck (Waves only) or Customer Service team member at Reception. These areas are consistently staffed during opening areas. Lifeguard staff (Waves only) or Reception are the primary point of contact for first aid assistance.
Has the visitor been explained any other rules, procedures, or information relevant to the visit?(Required)
Safe Working Safety Signage- Please be aware of, and follow any safety signage located around the facility, it is there for your safety. Restricted Areas- Rooms marked ‘Staff Only’, ‘No Entry’ or ‘Restricted Entry’ should only be accessed at the approval of City Staff. Plant and Equipment- Use of plant and equipment including isolation and de-isolation is only permitted at the approval of City staff. This site uses ‘Out of Service’ and ‘Danger’ tag out systems. Public Areas- Please be caution of public users when working in public areas. Cordon off areas to isolate public from hazards where appropriate. Photography is permitted with the approval of City Staff. PPE- Contractors and visitors are required to supply, maintain and use appropriate PPE for the required task. Permit to Work- Relevant to this facility, The City operates a permit to work system when working; at heights (>2m), with chlorine gas, with high voltage, in confined spaces, with hot works and/or near traffic. Other works may require permits. The issuing Officer will advise the process where required.
