Please ensure you read the following information carefully before submitting your application:

• Confirmation of bookings (and amendments) are subject to ground availability
• Once your booking is confirmed by the City of Bayswater, a contract and tax invoice will be issued. Payment is due immediately on receipt.
• Booking amendments must be submitted by email to recreation.services@bayswater.wa.gov.au Fees may apply for cancelled bookings subject to the ‘Seasonal Ground Use Conditions – Winter 2026
• If you do not currently have a permit from the Department of Racing, Gaming and Liquor to serve or consume alcohol, you must first contact the City of Bayswater for approval. Please email recreation.services@bayswater.wa.gov.au or phone the City on 9272 0992.

I have read, understood and accept the terms, conditions and general information specified in the Winter Season 2026 Ground Application Form, and the Seasonal Ground Use Conditions – Winter 2026.

I understand that it is a condition of hire that all clubs using City of Bayswater reserves and facilities for sport activities must hold Public Liability Insurance of $20 million and adhere to applicable Working With Children Legislation.

I have attached a copy of the club’s Public Liability Certificate of Currency to this form.

I give consent to the City of Bayswater to publish Office Bearers’ contact details in appropriate documents and directories.
